We’re here to help, 24 hours a day, 7 days a week.

You can call your local Luminary Hospice office anytime—or, if you prefer, simply fill out our referral form, and a member of our team will follow up promptly to begin the process.

Hospice services are available wherever the patient calls home. We’ll be happy to confirm availability and answer any questions you may have.

Once a referral is received, a Luminary team member will reach out to all appropriate family members or care providers to schedule a consultation and/or patient assessment, ensuring a smooth and respectful transition into care.
