The Hospice Journey: Step by Step

Before any decisions are made, we begin by listening. Many families start with a simple phone call, just to ask questions or understand if hospice might be the right next step. Whether you’re unsure about eligibility or just need someone to talk things through with, our team is here to guide you with clarity and compassion, without pressure.

When hospice care begins, our team meets with the patient and family to learn about needs, explain services, and answer questions. Together, we review medications, equipment, and goals of care, helping families feel informed, supported, and prepared.

Hospice care is never one-size-fits-all. We work closely with families to create a plan that reflects the patient’s physical, emotional, and spiritual needs, guided by values, preferences, and what matters most. This plan evolves as needs change.

 

Ongoing Care & Visits

A dedicated care team, including nurses, aides, social workers, chaplains, volunteers, and others, provides regular visits wherever the patient lives. Care is adjusted as symptoms or circumstances change, with a continued focus on comfort and quality of life.

 

Support for the Caregiver

Caregivers are an essential part of this journey. Our team offers education, coaching, and emotional support to help loved ones feel more confident and less alone, whether that means learning how to manage medications or having someone to talk with during difficult moments.

 

Extra Support During Times of Change

When symptoms or needs change, additional levels of care may be provided, including Continuous Care or General Inpatient Care. These short-term supports offer more intensive attention during periods of transition, helping ensure comfort, stability, and peace of mind, while avoiding unnecessary hospitalizations whenever possible.

As the final days approach, care centers on comfort, dignity, and presence. Our team helps families understand what to expect, offers guidance through physical and emotional changes, and remains closely alongside loved ones during this time.

Care does not end with a loved one’s passing. Luminary supports families with immediate needs, offers space for grief, and continues to walk alongside loved ones with bereavement support, including counseling, resources, and compassionate check-ins.
