Around-the-Clock Support During Medical Crisis

When a patient is experiencing intense symptoms or a sudden change in condition, continuous care provides short-term, in-home support to bring stability and comfort without requiring a hospital stay.

This level of care ensures that patients can remain in familiar surroundings while receiving close, attentive monitoring and symptom management.

When Continuous Care Is Needed

During moments when symptoms become overwhelming or unpredictable, continuous care offers focused attention to bring rapid relief and restore stability. Continuous care may be recommended when a patient experiences:

  • Severe pain or discomfort
  • Difficulty breathing
  • Uncontrolled nausea or vomiting
  • Anxiety or agitation
  • A rapid decline in condition requiring close observation

These situations can feel frightening to patients and families, and continuous care provides the steady presence and clinical support needed to feel safe and supported.

What Continuous Care Care Includes

Continuous care provides extended, bedside support designed to bring comfort and control during a medical crisis. This level of care includes:

  • Licensed nurses at the bedside for a minimum of 8 hours per day, up to 24 hours as needed
  • Support from hospice aides, social workers, and chaplains
  • Immediate access to medication adjustments and care interventions
  • Emotional support for the patient and loved ones during a vulnerable time
  • Coordination with physicians to manage symptoms and guide next steps

Each service is delivered with urgency and compassion, ensuring the patient and family never face these critical moments alone.
