Referring a patient to Luminary Hospice is designed to be straightforward and responsive.

Our teams are available 24/7 to receive referrals, coordinate timely assessments, and begin services as appropriate, while maintaining clear communication with referring providers throughout the process.

What to Expect After Referral

Once a referral is received, a Luminary Hospice team member initiates outreach to the appropriate family members or care providers to coordinate next steps. This may include scheduling a consultation and, when appropriate, a patient assessment.

Throughout this process, Luminary works collaboratively with the referring provider and care team to ensure communication is clear, roles are respected, and the transition into hospice care is handled with care and professionalism.
